29 CFR 1910.177(g)(8): The employer did not establish a safe operating procedure to ensure that employees stay out of the trajectory when inflating a tire on a single piece rim wheel: (a) 820 Petersburg Road, Carlisle, PA - The employer did not establish an operating procedure to ensure that employees were out of the trajectory path when inflating tires on single piece rim wheels. On July 10, 2017, two employees suffered fatal injuries while servicing a single piece rim tire when the tire exploded while the employees were standing in the trajectory path. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
