29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On 6/15/17, at the Winder facility, Authorized employees did not maintain control of lockout devices once they were applied. Lead persons were observed locking out equipment and then handing the key to the operator of the machine.
