29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about June 13, 2017, at all three plant locations, there were no energy control procedures developed or utilized for multi energy source equipment. Employees perform maintenance, unjamming, tool changes, and clean-out at various machines. There were numerous multi energy source equipment that did not have detailed procedures developed to identify energy sources. Employees were exposed to the hazard of being caught-in the machine and/or an electrical shock hazard from an unexpected start-up of the machine. Energy control procedures were not developed for such machines as, but not limited to: a) Detail aisle, Kinetic 3000, plasma cutter, has such energies to include gas, heat, electrical. b) Detail aisle, Kinetic K5000XMC, plasma cutter, has such energies to include gas, heat, electrical. c) Detail aisle, Quick Mill (drill press), has such energies to electric and hydraulic. d) Detail aisle, back room, Ficep machine, has such energies to include electrical, hydraulic and pneumatic. e) Detail aisle, punch press, #86-024, has such energies to include electrical and hydraulic. f) Detail aisle, Kingsland punch, has such energies to include electrical and hydraulic. g) Detail aisle, AccurPress break, has such energies to include electrical and hydraulic. h) Wysong press break, has such energies to include electrical and hydraulic. i) Beamline, HE-M saw, has such energies to include electrical, hydraulic and kinetic (secure saw blade). j) Beamline, CNC Drill, Control Automation Inc., has such energies to include electrical and hydraulic. k) Girder plant and Art street plant, Squeezer machines, has such energies to include electrical and hydraulic.
