29 CFR 1926.800(d): Employees were not instructed in the recognition and avoidance of hazards associated with underground construction activities including: a) Culvert Install Site: On or about May 05, 2017 and at times prior, employees were exposed to acute illness, burns, impairment of ability to self-rescue, and incapacitation while removing soil, inspecting, tacking and welding inside of a 36 inch culvert. The employer failed to instruct the employees in the recognition and avoidance of hazards associated entering the culvert without monitoring the air. b) Culvert Install Site: On or about May 05, 2017 and at times prior, employees were exposed to acute illness, burns, impairment of ability to self-rescue, and incapacitation while removing soil, inspecting, tacking and welding inside of a 36 inch culvert. The employer failed to instruct the employees in the recognition and avoidance of hazards associated entering the culvert without and adequate ventilation system. c) Culvert Install Site: On or about May 05, 2017 and at times prior, employees were exposed to acute illness, burns, impairment of ability to self-rescue, and incapacitation while removing soil, inspecting, tacking and welding inside of a 36 inch culvert. The employer failed to instruct the employees in the recognition and avoidance of hazards associated entering the culvert without adequate communication to summons rescue in the event of an emergency. d) Culvert Install Site: On or about May 05, 2017 and at times prior, employees were exposed to arc flash and thermal burns while tacking and welding inside of a 36 inch culvert. The employer failed to instruct the employees in the recognition and avoidance of hazards associated entering the culvert without adequate fire resistant clothing to protect them from arc welding flash and thermal heat. e) Culvert Install Site: On or about May 05, 2017 and at times prior, employees were exposed to arc flash and thermal burns while tacking and welding inside of a 36 inch culvert. The employer failed to instruct the employees in the recognition and avoidance of hazards associated with entering the culvert without an adequate understanding of fire prevention and protection.
