29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinder is designed to accept a cap, shall always be in place, hand-tight, except when cylinders are in use or connected for use. 101 North Main Street, Newark, Illinois, west building shop, east entrance: on or about May 4, 2017, stored Acetylene cylinders did not have valve protection caps installed and were not prevented from falling and being struck-by material handling operations in the immediate area. Employees working in the area were exposed to struck-by hazards resulting from physical damage to pressurized cylinders. There is no abatement documentation or certification required for this item.
