29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that are likely to cause death or serious physical harm to employees such as damaged insulation. On or about April 20, 2017: a) In Building C employees engaged in detailing cars used vacuum cleaners. The vacuum cleaners received power from flexible cords that were damaged such that bare and insulated conductors were exposed, exposing employees to electrical shock.
