29 CFR 1910.68(b)(8)(iii): Gate(s) were not self-closing: a) The employer did not ensure that the gate used to protect employees from falling through the approximately 32 and ¾ inches in diameter hole for the downward side of the man-lift located in the grain cleaning room were self-closing. On April 24, 2017 the gate was open exposing employees to a fall hazard of approximately 16 feet to the basement below. b) The employer did not ensure that the gate used to protect employees from falling through the approximately 32 and ¾ inches in diameter hole for the upward side of the man-lift located in the grain cleaning room were self-closing. On April 24, 2017 the gate was wire tied open exposing employees to a fall hazard of approximately 16 feet to the basement below.
