29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: a) Warehouse/Facility: On or about April 19, 2017, and at times prior, the employer failed to conduct the required personal protective equipment (PPE) hazard assessment of the work areas to ensure employees were provided adequate PPE such as but not limited to steel toed shoes/boots and hard hats in the warehouse and shipping areas which exposed employees to the hazards associated with dropping heavy items onto their foot or getting ran over by the forklift and getting struck in the head while working under employees retrieving material from storage bens using forklifts or pickers.
