29 CFR 1910.146(i)(4): The employer did not ensure that each attendant remained outside the permit space during entry apparitions until relieved by another attendant. On March 27, 2017, the employer or his representative did not ensure that an attendant remained outside the confined space during the entire entry operations. This condition exposed employees to caught-in and struck-by hazards.
