29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. On or about February 21, 2017, at Blanchard Refining Company, LLC in Texas City, Texas; the employer failed to ensure maintenance employees received training in the recognition of the type and magnitude of the energy sources, and the methods and means necessary for energy isolation and control prior to decouple crude charge pumps such as charge pump J-27.
