29 CFR 1910.1048(k): When there was the possibility of an emergency involving formaldehyde, the employer did not ensure that appropriate procedures were adopted to minimize injury and loss of life and implemented in the event of an emergency: a) At the facility, where the employer did not ensure that there were emergency procedures available for employees handling formocresol. An employee dropped a bottle of formocresol while moving the bottle to a different location. The formocresol bottle was 37 percent formaldehyde.
