29 CFR 1910.146(h)(3): The employer did not ensure that all authorized entrants communicated with the attendant as necessary to enable the attendant to monitor entrants of the need to evacuate the space as required by 29 CFR 1910.146(i)(6): a. On or about January 14, 2017, in the phosphate area, employees (authorized entrants) were required to work inside a wash tank, a permit required confined space. The attendant spoke English and the authorized entrants spoke Spanish. The employer did not ensure that employees working as authorized entrants were able to communicate with the attendant as necessary.
