29 CFR 1926.859(g): During demolition, continuing inspections by a competent person were not made as the work progresses to detect hazards resulting from weakened or deteriorated floors, or walls, or loosened material. Employees were permitted to work where such hazards exist until they are corrected by shoring, bracing, or other effective means: a) Five Story Building: On or about February 14, 2017 and at times prior thereto, the employer did not ensure bracing, shoring, or other effective means were used when a portion of the fourth level wooden floor was weakened by the removal of supporting structures which exposed employees to death and other injuries associated with falls or being struck-by and crushed-by collapsing and falling portions of the structure.
