29 CFR 1910.334(a)(3)(i): A flexible cord used with grounding type equipment did not contain an equipment grounding conductor. The employer is failing to protect employees from electrical and explosion hazards associated with using ungrounded equipment in a combustible dust environment. The most recent occurrence of this was evident during the inspection of the facility located at 1700 E. Front St. Fremont, NE, where the employer has installed a Pelonis Convection Oil Filled Radiator Heater as means of providing heat in the load out control shed where the presence of combustible grain dust is evident during every delivery.
