29 CFR 1910.219(c)(4)(i): The employer did not ensure shaft ends projecting more than one-half the diameter of the shaft were guarded by nonrotating caps or safety sleeves: Peter's Machine Line 3- On or about January 31, 2017 employees were exposed to caught-in hazards while removing defective cookie wafers from belt. Employees were standing near three unguarded rotating shaft ends that were 1 inch diameter and projected from bearings approximately 2 inches. No documentation required for this item.
