29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) Wyvern Site, Track Location OOC 418.1 - On or about January 24, 2017, the employer exposed employees to an engulfment hazard, in that employees working in an unprotected trench ,ranging from 6 to 9 feet deep with walls that had been cut almost vertically, were not provided with cave-in protection. The near vertical excavated walls were not sloped, shored, or shielded.
