29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) The iron worker, an Original Peddinghaus, used to punch holes in metal bars, had no guard at the punch. (b) The iron worker, an Original Peddinghaus, used to shear metal, had a partially unguarded shear blade and the fixed metal barrier guard was missing fasteners and was loose. (c) The approximately 16 torque machines in the MSA area had rotating parts with no guard to prevent employee exposure.
