29 CFR 1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as de-energized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that the circuit elements and equipment parts were de-energized: a) 115 East Centre Street, In the Imaging Room, Nutley, NJ. Employee was exposed to electric shock and burns as the employer failed to ensure that a qualified person used test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verity that the circuit elements and equipment parts were de-energized.
