29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: a) On or about November 10, 2016 at Liquid Environment Solutions, Mobile, Al, railcar department: The employer failed to ensure the Toyota forklift in the railcar department was taken out of service for repairs for items such as but not limited to the horn not working, rubber brake pad missing, nameplate unreadable, seat belt sometimes locks during testing, and the owners manual missing.
