29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: a. On or about November 29, 2016, at the plant, second floor of the plant on the southeast side, the employer did not provide safety guards to prevent operators from coming into contact with the moving chains on the pay-off accumulator machine MV line # 2, employees were exposed to amputation hazards. b. On or about November 29, 2016, at the plant, second floor of the plant on the southeast side, the employer did not provide safety guards to prevent operators from coming into contact with the moving chains on the pay-off accumulator machine MV line # 1, employees were exposed to amputation hazards. c. On or about November 29, 2016, at the plant, first floor of the plant on the north side, the employer did not provide safety guards to prevent operators from coming into contact with the moving chains on the take-up side of stranding machine # 1, employees were exposed to amputation hazards.. d. On or about November 29, 2016, at the plant, first floor of the plant on the northeast side, the employer did not provide safety guards to prevent operators from coming into contact with the moving chains on the motors of the re-spool machine, employees were exposed to amputation hazards.
