29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch: The employer is failing to protect employees from struck-by hazard. This was most recently documented at a On 11/17/16 at the establishment located at 15 SW. 40 Ave in Great Bend KS. Employees were exposed to struck-by hazards while operating bench grinders with improperly adjusted tongue guards exceeding one-fourth inch opening. a) In the Machine Shop bench grinder (Property #04133) set to approximately one and one-eight inch opening. b) In the welding shop, Sears Craftsman Bench Grinder (Model 257.190430) tongue guard, set to approximately one inch opening. c) In the maintenance shop Grinder (Model #334) missing safety tongue guard, approximately one-half inch opening.
