Text For Citation: 01 Item/Group: 005 Hazard:
29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she began work, and did not remove those devices when he or she stopped working on the machine or equipment being serviced or maintained: (a) On or about February 14, 2017 and at times prior thereto, at least three maintenance employees repaired a foil press machine without each applying a separate lockout device, exposing employees to hazards associated with the unexpected release of hazardous energy; (b) On or about February 21, 2017 at least one maintenance employee and the maintenance supervisors repaired a Thompson clam shell without each applying a separate lockout device, exposing employees to hazards associated with the unexpected release of hazardous energy.
