29 CFR 1926.416(b)(1): The employer did not ensure that barriers or other means of guarding shall be provided to assure that work space for electrical equipment will not be used as a passageway during periods when energized parts of electric equipment are exposed: a) The employer did not have the aisle/work are barricaded to keep the passageway clear when energized electric panels DPH-40B-2 and 40-B-4 were being worked in. An arc flash occurred while work was being done in the energized panels. Location: 388 Greenwich St, New York, N.Y., 40th fl mechanical rm. On or about 10/08/2016 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
