29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a) 312 Old Goff Mountain Road: On or about October 12, 2016 and times prior, an employee performed framing work from the bucket of a 460SJ JLG aerial lift, approximately 15 feet 9 inches from the ground, with no fall protection.
