29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: The employer did not ensure portable ladders were constructed and placed so three (3) feet or more was extended above the upper landing. Employees performing residential roofing operations were exposed to falling approximately ten (10) feet while using an extension ladder to ascend/descend to the roof. The employer (Luiz Arnaldo Zayas Torres) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1115282, citation number 1, item number 6 and was affirmed as a final order on 03/28/2016, with respect to a workplace located at next to 218 Arbor Meadows Ct., Fairview Heights, IL 62208.
