29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, are not always in place, hand-tight, except when cylinders are in use or connected for use: Employees in the fabrication shop were exposed to fire and struck-by hazards and the employer fails to ensure that the valves on the compressed gas cylinders are protected by valve protection caps. This violation was last observed on September 1, 2016. To abate this violation, the employer must ensure that all cylinders that are designed to accept valve protection caps are equipped with caps, except when they are in use. Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including a copy of site-specific program detailing the procedures used to ensure compliance and training of employees.
