29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) On or about August 24, 2016, the employer did not ensure press operators were trained as authorized employees prior to performing die changes on the mechanical power press such as, but not limited to, Prestole Press #4 and Press #29. Employees were exposed to crushing hazards while performing this operation.
