Text For Citation: 01 Item/Group: 005 Hazard:
29 CFR 1910.184(d): Each day before being used, the sling and all fastenings and attachments were not being inspected for damage or defects by a competent person designated by the employer: On or about August 11, 2016, and at times prior thereto, at this metal molds and dies manufacturing plant production area, where employees were exposed to struck-by and caught-in/between hazards, associated with hoisting heavy equipment and molds with overhead cranes. The employer did not perform any complete daily inspections for the sling and all fastenings and attachments used for overhead cranes such as, but not limited to: a. Swivel eye bolts while lifting a metal mold; and b. Swivel eye bolts by rigging equipment station were available for use.
