29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: (a) Pipe Plant - On or about August 4, 2016 employees operating industrial fork lift trucks had not received and documented training evaluation once every three years. Forklift operators were last evaluated in 2008.
