29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): Excavation Site: On or about July 20, 2016, the employer did not provide an adequate protective system for employees working in the excavation that was approximately 172-feet 8-inches in length and 5 to 6-feet deep. This condition exposed employees to cave-in hazards.
