29 CFR 1926.403(b)(1): The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: (a) Pro-Tec Roofing, Inc. @ 1515 15th St. NE, Watertown, SD 57071: On or prior to April 21, 2016, the exposing employer did not ensure adequate insulation on an extension cord as it was missing approximately 3 inches of outer sheath insulation near the female end. This condition exposed the employee to hazards such as electrical shock and burns.
