29 CFR 1910.1200(h)(3)(ii): Employee training did not include the physical and health hazards of the chemicals in the work area: At Simmons Food, Inc.: The employer failed to train employees on the physical and health hazards of combustible dust material. Employees were working with and handling combustible dust material such as, blood meal. On March 26, 2016 at approximately 11:45 pm, an explosion occurred at the ProCal area of the establishment, located at 10700 South State Highway 43, Southwest City, Missouri 64863. Three employees were exposed to fire and explosion hazards when blood material was over dried during the recycle stage of the process.
