29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified per 29 CFR 1910.178(l): (a) Ark Woodworks Inc, at 878 Baseline Place, Brighton, CO: On and preceding 4/4/16, the employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified per 29 CFR 1910.178(l). Employees operated a Nissan 50 model CPF02 powered industrial truck while transporting materials. The employees had not completed training and evaluation with regards to competency to operate the Nissan powered industrial truck. This condition exposed employees to a powered industrial truck hazard.
