29 CFR 1910.219(c)(4)(ii): The unused keyway(s) of projecting shaft end(s) were not filled up or covered: (a) Freeze tunnel, control station side: On or about February 11, 2016, employees were exposed to a caught by hazard, while walking by the freeze tunnel that had an exposed rotating shaft with a keyway. (b) Fry line, raw side flour tumbler applicator station: On or about February 12, 2016, employees were exposed to a caught by hazard, while walking and working on the flour tumbler station that had an exposed rotating shaft with a keyway. (c) Fry line, raw side, east: On or about February 17, 2016, employees were exposed to a caught by hazard, while walking by the fry control station that had an exposed rotating shaft with a keyway.
