29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: (a) 101 North Main Street, Newark, Illinois, west building shop, Cincinnati mechanical shear #20 model no. 2510, serial no. 19745 - on or about February 23, 2016, employees were exposed to contact with live electrical wiring at an electrical box that had a missing cover while picking up sharp, scrap metal pieces that were thrown on the floor and when turning on/off overhead light switch in close proximity in close proximity to exposed, energized wires. (b) 101 North Main Street, Newark, Illinois, west building shop, SteelWeld 500 Ton mechanical press brake model no. M06-12 - on or about February 23, 2016, employees were exposed inadvertent contact with live electrical wiring at an electrical box that had a missing cover while picking up sharp, scrap metal pieces that were thrown on the floor in close proximity in close proximity to exposed, energized wires. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
