29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group: The employer is failing to protect employees from the hazards of unexpected machine activation or energization during maintenance operations involving multiple persons. This was most recently documented on February 15th, 2016, at the Stahl Specialty Company facility located at 1301 Stahl Drive in Warrensburg, Missouri, in the Sand Core Department where an outside contractor was assisting in performing non-routine maintenance on a Laempe LFB50-H Sand Core machine. No procedure was utilized to afford employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device.
