Mattocks Five, Inc. dba Albion Mill, 73 Canal Street, Albion, PA 16401, on or about February 10, 2016 and times prior thereto: Employees operate a Rollo-Mixer brand batch mixer to make product including, but not limited to, animal feed. The rotating batch mixer drum is powered via an unguarded chain and sprocket wheel. The unguarded chain / sprocket was located within 2-feet of the floor.
