29 CFR 1910.27(b)(3): Fastenings were not an integral part of the fixed ladder design: A.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA, 15824, production area, on or about November 23, 2015 and times prior thereto: Employees climb up and down a fixed ladder below grade at the Gasbarre 60T powered metal press (M9) to perform maintenance activities including, but not limited to, removing and replacing core rods for a second Gasbarre 60T powered metal press (M5). The fixed ladder was not bolted to the wall of the pit.
