29 CFR 1910.252(a)(2)(xiv)(E): The employer did not ensure that the supervisor determined that the cutter or welder secured his approval that conditions were safe before going ahead. On or about November 7, 2015, employees engaged in the use of oxygen and acetylene cutting torch operations were exposed to explosion hazards, did not secure approval that the conditions were safe before proceeding.
