Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by flying fragments as result of an explosion as a result from inadequate boiler controls. Feasible abatement would be to comply with the National Fire Protection Association, NFPA 85A, Standard for Prevention of Furnace Explosions in Fuel Oil- and Natural Gas-Fired Single Burner Boiler-Furnaces. 1)Provide a convenient observation port on the control side that will allow the fireman to visual inspect the main burner flame or other applicable procedures, such as, utilizing 2 employees during boiler start-up. 2)Provide a reliable control to shut off main flame fuel if ignition has not occurred within 10 seconds. 3)Provide a control that will verify that the supervisor gas cock is closed before starting the purge system. 4)Provide a control that will initiate a safety shutdown upon detection of low gas pressure.
