29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: (a) The employer did not ensure that authorized employees had received training in the methods and means necessary for energy isolation for equipment such as, but not limited to, the Milnor dryer and Pellerin Milnor conveyor. On or about August 13, 2015, an employee was propelled into an industrial dryer when the conveyor system activated. The dryer subsequently activated with the employee trapped inside. (b) The employer did not ensure that authorized employees had received training in the methods and means necessary for energy isolation for equipment such as, but not limited to, the Milnor dryer and Pellerin Milnor conveyor. On or about August 11, 2015, an employee was conducting maintenance work without using proper hazardous energy control procedures.
