Text For Citation: 01 Item/Group: 005 Hazard:
29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: The employer is failing to train employees to recognize the fall hazards associated with using a portable ladder as means of access and egress to upper landing surfaces on construction sites. The most recent occurrence of this took place the job site located at 416 N. Jeffers St. in North Platte, Nebraska, where the employer had inadequately trained employees using a Keller 25 foot aluminum extension ladder to access the decking the company was installing. Examples of the inadequate training include but are not limited to: a. On August 3, 2015 an employee using the damaged 25 foot Keller aluminum ladder as means of access/egress to the upper decking. b. On August 3, 2015 an employee using the 25 foot Keller aluminum ladder as means of access/egress to the upper decking without extending the ladder a minimum of three feet over the upper landing surface. c. On August 3, 2015 an employee carrying 3-4 2 foot long steel angle iron while using the 25 foot Keller Aluminum ladder as means of access and egress to the upper decking. d. On August 11, 2015 an employee using the damaged 25 foot Keller aluminum ladder as means of access/egress to the upper decking. e. On August 11, 2015 an employee using the 25 foot Keller aluminum ladder as means of access/egress to the upper decking without extending the ladder a minimum of three feet over the upper landing surface.
