29 CFR 1910.177(g)(8): The employer did not ensure that employees stay out of the trajectory when inflating a tire on a single piece rim wheel: a) BK Tires, Inc. at 3775 Puritan Way, Frederick, CO 80516: On or before July 16, 2015, the employer failed to ensure that employees stayed out of the trajectory when inflating a tire on a single piece rim wheel. Condition exposed employees to projectile and struck-by hazards, and resulted in the fatality of an employee while he was inflating a 315/80R22.5 truck tire.
