29 CFR 1910.146(h)(2): The employer did not ensure that all authorized entrants properly used equipment as required by paragraph (d)(4) of this section: On or about October 12, 2011, in the facility, employees were required to enter permit spaces, including, but not limited to, ISO containers, during cleaning and maintenance operations. Potential hazards in permit spaces included toxic and/or flammable and combustible atmospheres from residual (heel) and spot cleaning chemicals. The employer did not ensure that all authorized entrants properly used required equipment including testing, monitoring and rescue equipment.
