29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: a/ At the Main Building, specifically at the Secondary Processing Area, which consisted of two loading docks at either side of the facility, protective helmets were not provided to employees who operate overhead cranes where the suspended load was at or above their heads.
