29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: a) On or about 05/08/15 in the Textile Department; employer failed to protect employees who operate the core cutting machine ( #689452-G2 ), from being able to access the cutting blades, when one of the guards was missing an attachment device which allow the guard to be repositioned allowing access to the point of operation and exposure to an amputation hazard. NO ABATEMENT DOCUMENTATION REQUIRED
