29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer failed to ensure that various equipment or machinery used at the site was effectively guarded. The unguarded and/or inadequately guarded equipment includes; but is not limited to, the following: a. The hot metal circular saw located in the roll shop was not adequately guarded. Employees are exposed to flying chips and sparks. b. The American (vertical) drill and boring mill was unguarded. Employees are exposed to potential rotating parts. c. Rockford cutter and milling bed, located in the machine shop, was unguarded. Employees are exposed to potential struck- by hazards. d. The Giddings and Lewis boring, drilling and milling machine shop, was unguarded. Employees are exposed to potential struck-by hazards. e. The platen bed of the conveyor system of the Kearney & Trecker milling machine was unguarded, exposing employees to in-going nip points and/or crushing hazards. f. The rotating chuck of the Le Blond lathe, located in Equipment Rebuild, did not have a chuck guard. Employees are exposed to an entanglement hazard.
