29 CFR 1910.27(f): All ladders shall be maintained in a safe condition. All ladders shall be inspected regularly, with the intervals between inspections being determined by use and exposure: a) The employer did not ensure that fixed ladders were maintained in a safe condition. This violation occurred on or about June 3, 2015 when the fixed metal ladder over tuber #6 was damaged and bent.
