29 CFR 1926.1053(b)(16): Portable ladders with structural defects were not immediately marked "Defective, Do not Use" and withdrawn from service until repaired: a) The orange (fiberglass) extension ladder that was being used for employees to go up to the 6th floor was missing an extension ladder hook. Employees were exposed to fall hazard. Location: 360 Neptune Ave, Brooklyn, N.Y. On or about 4/1/2015 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
