29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) RM4 AMADA Band Saw - The AMADA band saw blade was not adequately guarded during operation. A nine and one half inch (9 1/2") section of the blade on the left side was exposed because the clamp was fixed and not adjustable. A twenty-one and one half inch (21 1/2") section of blade on the right side of the clamp was exposed, the guard was flipped up because it hit the clamp when it was down. Employees were exposed to amputation hazards, on or about May 6, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
